The End of an Era: The Iconic “Man in the Iron Lung” Dies at 78

For over 70 years, Paul Alexander lived inside a 600-pound metal cylinder, yet he somehow managed to touch more lives than most people ever will. Known globally as the “Man in the Iron Lung,” the Dallas native passed away on March 11, 2024, at age 78, leaving behind a legacy of resilience that defied the limits of modern medicine.

His journey began in 1952, during a devastating polio outbreak in Dallas, Texas. At just six years old, Alexander was paralyzed from the neck down, unable to breathe on his own. Doctors placed him in an iron lung—a mechanical respirator that used negative pressure to force his lungs to expand. While most patients during that era spent only a few weeks in the machine, Alexander’s paralysis was permanent. He remained in that iron lung for the next seven decades.

A Life Without Limits
While his body was confined to a metal tube, Alexander’s ambitions knew no bounds. He became a symbol of what the human spirit can achieve against impossible odds. He famously learned to memorize his studies to graduate second in his high school class—without ever attending a physical class. He went on to earn a law degree from the University of Texas, passing the bar exam and practicing law in a three-piece suit while propped up in a modified wheelchair.

“He wanted to change the world,” his brother, Philip Alexander, told CBS News. “He wasn’t going to leave until, in his mind, he did something grand.”

What We Know
Diagnosis: Contracted polio at age 6 in 1952.

Longevity: Recognized by Guinness World Records for the longest-ever duration of iron lung usage (over 70 years).

Achievements: Published his memoir, Three Minutes for a Dog, using a stick held in his mouth to type.

Final Chapter: Became a TikTok star in 2024, using his platform to offer messages of hope to hundreds of thousands of followers.

Status: He was one of the last two people in the world known to rely on an iron lung, alongside Martha Lillard.

The Final Breath
In his final months, Alexander reached a new generation through TikTok, where he shared his unfiltered reality—the isolation, the joy, and the constant struggle of his condition. Even after being hospitalized with COVID-19 in February 2024, his spirit remained unbroken.

“The night before he died, he just opened his eyes and looked at me and said, ‘This is perfect,'” his brother recalled.

Why This Matters
Paul Alexander’s life serves as a stark reminder of the fragile victories of public health. His existence was a living history of the polio era, a time when thousands of families were torn apart by a disease now largely eradicated by vaccines. His passing marks the close of a chapter in medical history, but his story resonates today as a powerful lesson in perspective. In a world often caught in the trivial, Alexander proved that even from the confines of a steel cylinder, one can reach out and hold the world’s attention.

His life was a testament to the fact that while we cannot always control our circumstances, we retain complete authority over our resolve. He didn’t just survive his machine; he redefined what it meant to live.
